TENURE AND PROMOTION FORMS
College of Arts and Sciences

Please include the following five forms in your T&P dossiers.

• Appointment History Form
• CV Cover Sheet
• Tenure & Promotion Recommendation Form (Inter-Office Communication)
• SIRS Summary Form
• SETE Summary Form

1. The Appointment History form goes in TAB I of the dossier.
2. The CV Cover Sheet goes in TAB IX on top of your university CV.
3. The Tenure and Promotion Recommendation Form goes in Tab I
4. The SIRS Summary goes in TAB VI behind your teaching philosophy statement. This is the updated form (rev. 7/04 - this form is different than the one currently in the Example Dossier and on the Provost's webpage). This form is necessary because the SIRS summary reports you receive each semester changed in Spring '04. The updated form has two sections: SIRS prior to Spring '04 and SIRS Spring '04 & Beyond.
5. SETE Summary form goes in Tab VI behind the SIRS Summary form.

Dean Kurtz requires FIVE years of SIRS/SETE information in the summary sheet (if you have been at the UofM five years or longer). If you have been here less than five years, provide summary info for all courses taught here.
